PIAA Participation Requirements: Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests at any PIAA member school in any school year, the student is required to complete a Comprehensive Initial Preparticipation Physical Evaluation (CIPPE) by doing the following:

You will need to upload a PIAA Physical Form (Section 6) for each school year that you participate in a sport. Please upload Section 8 Parent Recertification if your child played a fall or winter sport. Click here for the PIAA Physical Form.
