PIAA Participation Requirements: Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests at any PIAA member school in any school year, the student is required to complete a Comprehensive Initial Preparticipation Physical Evaluation (CIPPE) by doing the following:

*** SECTION 6 PHYSICIAN’S PHYSICAL (PIAA Physical Form Section 6) is the only page you need to print and upload to this registration. Section 6 must be filled out by an Authorized Medical Examiner (AME) and dated on or after May 1 before the start of the current school year and uploaded in the appropriate section of this online registration.
Complete all sections of the form.
(4) STUDENT ALREADY PARTICIPATED IN A SPORT THIS SCHOOL YEAR?
Make sure to login using the same email as you did in the previous season.
Once logged in, select the winter sport for the participant and select the button with the participant’s name. Review and make any necessary changes to the information provided previously.
Since you already uploaded a Section 6 Physician’s Physical for this school year and you logged in with the same email, the physical will already be attached, and you do not need to upload it again.
However, you may need to upload a Section 8 – Recertification by Licensed Physician of Medicine or Osteopathic Medicine.
If you competed in a sport already for the current school year, AND you have 1) sustained a serious illness and/or serious injury that required medical treatment from a licensed physician of medicine or osteopathic medicine, or 2) had a concussion (i.e. bell rung, ding, head rush) or traumatic brain injury, or 3) experienced dizzy spells, blackouts, and/or unconsciousness, or 4) experienced any episodes of unexplained shortness of breath, wheezing, and/or chest pain, or 5) are taking any NEW prescription medicines or pills, or 6) you have any concerns you would like to discuss with a physician, then you need to upload a Section 8, Re-Certification by Licensed Physician of Medicine or Osteopathic Medicine, within this online registration. If applicable, Section 8 is part of the PIAA Physical Form and must be filled out by an Authorized Medical Examiner (AME), and uploaded in the appropriate section of this online registration.
Complete the parent/guardian signatures as required in the remaining sections and submit the registration.
